By the end of this course, participants (including service users, carers, and healthcare professionals) will be able to:
- Clearly explain what an advance choice document is.
- Describe who should be involved in creating one, and understand their respective roles.
- Engage and empower individuals, even those who may be reluctant, in the process of making an advance choice document.
- Confidently complete an advance choice document using the provided template.
- Approach the completion of advance choice documents in a manner that is trauma-informed and respectful of the person’s protected characteristics, as well as their cultural and spiritual background.
- Implement a shared decision-making model that involves service users, carers, and healthcare professionals in the creation of advance choice documents.
- Identify potential complexities that may arise during the creation of advance choice documents and explore strategies to address these challenges.
